This was done in BANDWITH's rescaled solar system mod.
The launch vehicle I used for this is my old Saturn V, which worked better than I thought for this. Here's a screenshot for those who want to see it:
Now, the mission.

Orbit was achieved with the third stage of the rocket.
Lunar Encounter:
Lander and CM separation in Lunar orbit:
Landing:
CM and CM Rendezvous:
AM Deorbit:
Earth Re-entry path:
Re-entry (one thing to note here is that Earth's atmosphere is 10km tall in this mod due to it being 1/10 scale)
Parachutes deployed!
Mission Accomplished!
